A HMR helps patients to understand their medications and enhances their ability to manage their medications appropriately. Your could benefit if:

Who is Eligible?

Patients with any ONE of the following are eligible:

  • Taking 5 or more medicines
  • Recent changes in medications or discharged from hospital
  • Taking a medication that needs to be closely monitored
  • Suffering a potential side effect
  • Sub-optimal response to a drug
  • Possible problems with managing all the tablets and devices
  • Language difficulties, dexterity problems, impaired sight or confusion

Frequently Asked Questions

How much does it cost?

Free on Medicare with a referral from your doctor, private consultations for non-Medicare patient’s available on request.

How long will the visit take?

On average about 45 minutes

Does it need to be at home?

No, but it is preferred. Alternative arrangement could be made with your GP or the pharmacist.

Why have I been referred?

Since you take more than 5 tablets, your GP feels this could potentially be a very beneficial service.

What happens with the pharmacist’s
recommendation?

Your GP will be sent a letter after the visit. You and the GP will then discuss any options in a follow-up appointment.

When can I have another visit?

Most patients will be offered a follow-up visit from their pharmacist between 6 weeks and 3 months following their first appointment.

The Process

  • 1. Your GP sends the HMR referral to the pharmacy/pharmacist
  • 2. The pharmacists phones you to arrange a convenient time for the visit
  • 3. The appointment at your home lasts around 45 minutes
  • 4. Your GP then has follow-up consultation with you to discuss any recommendations
